New Players (Those that have been offered spots on teams).  Must complete player registration and make payment in full or down playing and set up payment plan in order to guarantee roster sport and participate.

Guest Players playing on a team that is in our club.  This must be filled out prior to participation.

New and Returning Coaches (All head coaches need to fill this out at the beginning of the season).

Coaches need to be reimbursed for an expense or to request payment for a tournament, league, etc...
